Job summary

Your Aging Therapist, LLC is seeking compassionate Physical Therapists and Physical Therapist Assistants who help adults improve mobility, safety, independence, and quality of life through clinic and home-based care.

Applicants should hold GA PT/PTA licensure, prefer SC licensure, and be willing to travel to patient homes. CPR/BLS, a valid driver's license, and good documentation are essential. New graduates are welcome to apply.

Qualifications

• Licensure in Georgia (PT/PTA) in good standing.
• Licensure in Georgia and South Carolina preferred, or willingness to obtain SC licensure.
• Graduate of an accredited PT or PTA program.
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation, and appropriate auto insurance.
• Willingness to work in both clinic and mobile/home-based settings.
• Ability to travel to patients' homes and community locations as needed.
• Strong communication, organization, and time-management skills.
• Ability to work independently while collaborating with the therapy team.
• Timely and accurate completion of patient documentation.
• Comfortable working with older adults and variety of conditions.
• Ability to safely assist patients with transfers, gait, balance, exercises, and mobility.
• Commitment to patient-centered, compassionate care.

Responsibilities

• Experience with geriatric or adult rehabilitation
• Experience with home-based/mobile therapy
• Experience with fall prevention, balance, neurological, or orthopedic rehabilitation
