Domnovate in York, PA is seeking a Physical Therapist for a permanent, on-site opportunity in a 151-bed skilled nursing setting. The position is 40 hours per week, Monday–Friday with weekend rotation, day shift 8:00 AM–4:30 PM.

Your contribution

Evaluate functional needs and movement or mobility limitations, and develop individualized treatment goals. Deliver therapeutic interventions, reassess progress and adapt the plan of care as needs change. Use exercise, gait, balance, strength and functional mobility strategies to connect treatment with activities that matter to each person. Explain techniques clearly and help patients and caregivers participate safely in care.

Collaborate with the rehabilitation and care team so treatment goals connect with each person's daily activities. Document evaluations, interventions and progress accurately and promptly, communicate relevant changes, and follow professional standards, safety practices and applicable requirements. The aim is thoughtful, coordinated care that respects each person's abilities, priorities and dignity.

Schedule and employment
