Qualifications

Required:

  • Bachelor's Degree in Physical Therapy (PT)
  • Minimum of 6 months PT experience
  • Proficiency with PC skills

Preferred:

  • Doctorate in Physical Therapy

Licenses and Certifications:

  • Physical Therapist License
  • AHA Basic Health Care Life Support (BLS) Certification
Responsibilities

The therapist is responsible for providing professional rehabilitation therapy to patients in their homes. This includes evaluating patients, developing, and implementing individualized, goal-oriented therapy programs in accordance with organizational policies and external guidelines.

  • Complete patient evaluations within the designated timeframe, ensuring assessments are holistic and comprehensive.
  • Develop personalized treatment plans based on evaluation findings, focusing on measurable short- and long-term goals.
  • Prepare documentation such as progress notes, reassessments, and discharge summaries, ensuring they reflect skilled care and monitor patient progress.
  • Ensure documentation is accurate, timely, and supports continued skilled care needs.
  • Follow physician's plan of care regarding service frequency and duration.
  • Coordinate with physicians, MCPs, and the healthcare team to ensure effective communication, resource referral, and utilization of home health aides.
  • Supervise the Physician Therapy Assistant (PTA), assessing their performance regularly and ensuring co-signatures are completed within regulatory timeframes.
  • Educate patients and caregivers with written home programs, submitting copies to medical records as per policy.
  • Provide comprehensive education tailored to patient and caregiver needs in an understandable manner.
  • Build rapport and mutual respect with patients to foster an environment conducive to goal achievement. Implement treatment plans with clinical proficiency and meet productivity standards independently.
  • Participate in discharge planning and address any issues that arise.
  • Complete scheduling and documentation requirements accurately and promptly using system applications.
  • Respond to messages and voicemails timely and document communications appropriately.
  • Ensure discharge summaries accurately reflect patient status, outcomes, and recommendations.
  • Perform other duties or projects as assigned, demonstrating flexibility and adherence to organizational standards.
  • Lift, push, and pull a minimum of 50 lbs., and stand for at least 8 hours daily.
