Job summary

Northwell Health is seeking a per diem Physical Therapist in Fairfield County, CT. The role involves PT assessments, planning and delivering therapy per physician orders, and discharge planning.

Responsibilities include coordinating with the care team, identifying patient needs for additional home health services, and counseling patients and families. Licensure and a minimum of one year of experience are required.

Qualifications

• Bachelor’s degree.
• Minimum Experience: One (1) year
• Current Physical Therapist license

• Participates in case management conferences with team members to coordinate physical therapy and total plan of care.
