Job summary

Montage Health in California seeks a Per Diem Physical Therapist to provide services to patients of all ages in Post-Acute Services, including our Inpatient Rehabilitation Facility and Skilled Nursing Facility.

The therapist collaborates with a multidisciplinary team, conducts examinations, evaluations, diagnoses and interventions to reduce disability, and assists in prevention, wellness, program development and staff education.

Qualifications

• Experience with patient populations including neurological, orthopedic, and medical conditions preferred.
• Education and training sufficient to obtain a California Physical Therapist license.
• Current California Physical Therapy license required.
• American Heart Association Healthcare Provider BLS required.

Responsibilities

• Provide Physical Therapy services to patients of all ages to reduce disability.
• Collaborate with a multidisciplinary team in care of patients.
• Educate, direct and supervise physical therapy services and support personnel.
• Perform examination, evaluation, diagnosis, prognosis and intervention.
• Contribute to prevention and wellness services and assist in program development and teaching.

The Physical Therapist interacts and practices in collaboration with a variety of professionals involved in the care of the patients. The Physical Therapist educates and directs and supervises physical therapy services including support personnel. Physical Therapy services include examination, evaluation, diagnosis, prognosis and intervention of the patient in order to reduce disability. The Physical Therapist also provides prevention and wellness services and assists in program development and teaching to patients and staff.
