Job Details

The Staff Physical Therapist is responsible for the evaluation, treatment, and discharge planning of all patients referred for their services. The Physical Therapist is an integral healthcare provider accountable for helping patients reach their highest level of function. The Staff Level I Therapist will uphold the mission, vision, and values of Jefferson Health. They work in conjunction with other healthcare professionals to coordinate treatment plans and care.

Job Description

Essential Functions:

1. Provide therapy evaluations, treatment planning, interventions, and discharge planning for a full patient caseload in acute care, skilled facilities, acute rehab, outpatient settings as appropriate.
2. Participate in student education and training.
3. Plan, organize, and deliver treatment programs to help clients/patients recover and rehabilitate.
4. Utilize tools and techniques to administer safe therapeutic interventions and training for patients and families.
5. Record and document clients'/patients' health progress, including evaluations, daily treatments, progress notes, and discharge summaries.
6. Perform other duties as assigned.

Education:

• Bachelor's, Master's, or Doctoral Degree from an accredited Physical Therapy Program.
• 0-2 years of clinical practice required; 1 year preferred.
• Valid state license in Physical Therapy.
• CPR certification (American Heart Association for Healthcare Providers, BLS) required.
