Physical Therapist Job Summary

As the Physical Therapist, under the supervision of the HomeCare Rehabilitation Supervisor, you will be a vital member of a dynamic team responsible for organizing and administering prescribed physical therapy programs in an effort to reduce and/or prevent disability and to assist patients in achieving maximum functional potential.

Physical Therapist Job Responsibilities

• Evaluates and treats patients as prescribed by referring physician.
• Records treatment, response, progress and discharge information in patient’s electronic chart using portable electronic device.
• Educates patient, family and significant others regarding condition, treatment and home program.
• Routinely confers with physician and other practitioners in person, by phone, or in writing, regarding progress, regression or change of plan.
• Orients, instructs, and directs other Health Care Team members as needed.
  
  

Physical Therapist Education And Experience Requirements

• BS, MS or DPT degree from an accredited program in Physical Therapy required.
• Connecticut license or Connecticut license eligible required.
• CPR Certification required.
• Minimum of one year experience preferred.
• Valid Connecticut Driver’s License and proof of current insurance coverage required.
  
  

Physical Therapist Knowledge, Skills, And Abilities

• Uses common sense and special medical skills to care for the sick or handicapped.
• Understands technical information from supervisors, charts, reference books, manuals and labels.
• Ability to use Portable Electronic Device to record/maintain patient information pertaining to Plan of Care.
• Works fast and accurately in an emergency.
• Communicates with people when they are sick, handicapped or nervous.
• Keeps up to date on trends and developments in the field of physical medicine and health services.
