Job summary

Lehigh Valley Health Network is seeking a Physical Therapist for per diem Home Care assignments. The role evaluates, plans, directs, and administers physical therapy programs to restore function and help patients reach their maximum independence, following physician diagnoses and care plans.

You will perform assessments, develop plans of care, provide hands-on therapy and patient education, communicate progress to supervisors and care teams, and ensure documentation meets regulatory

Qualifications

• Bachelor’s Degree in Physical Therapy must be completed.
• American Heart Association BLS certification required upon hire.
• Licensed Physical Therapist in State of Pennsylvania upon hire.

Responsibilities

• Establishes and implements physical therapy plans of care to restore function and maximize performance.
• Implements individualized treatment plans using evidence-based techniques and mobility training.
• Provides therapy within patient care needs and reassesses as indicated.
• Communicates patient progress to supervisor and care team.
• Ensures documentation adheres to policies, standards and regulatory requirements.

Summary

Evaluates, plans, directs, and administers physical therapy programs to restore function, prevent disability, and help patients reach their maximum level of independence. Provides assessment of patient condition based on diagnosis from the referring physician. Develops and updates an appropriate plan of care in collaboration with the patient/family to assist in returning them to their prior level of function. Improves or facilitates quality of life through prescribed exercise, hands‑on care, and patient education. Helps people achieve fitness goals, regain or maintain their independence, and lead active lives.

Job Duties

• Establishes and implements appropriate physical therapy plans of care to restore function, prevent disability, and help patients reach their maximum performance level.


• Implements an individualized treatment plan through the use of skilled, evidence-based therapy techniques such as therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities.


• Provides physical therapy treatment consistent with the patients’ needs assessment.


• Performs reassessment when indicated and modifies physical therapy plans accordingly.


• Communicates patient progress and physical limitations to supervisor, physician(s), and other team members.


• Implements a physical therapy plan of care to individuals based upon age-specific and population specific needs utilizing a developmental and patient/family directed approach.


• Provides supervision in accordance with regulatory requirements, as appropriate.


• Establishes a physical therapy treatment plan with measurable goals and time frames.


• Provides cost effective care in accordance with departmental billing guidelines.


• Completes and submits all documentation based on policies/standards and regulatory requirements.

Minimum Qualifications

• Bachelor’s Degree Physical Therapy


• Ability to work at various locations per diem.


• Knowledge of best practices in physical therapy.


• Demonstrates sound clinical reasoning in synthesizing data gathered in the evaluation and identifying treatment diagnoses.


• Must be able to provide safe, compassionate, and competent physical therapy treatments.


• Must be able to communicate with patients, family, and co-workers in a friendly, supportive manner.


• Ability to travel to patient care assignments - Home Care


• American Heart Association Basic Life Support - State of Pennsylvania Upon Hire


• PT - Licensed Physical Therapist - State of Pennsylvania Upon Hire

Preferred Qualifications

• Master’s Degree Physical Therapy or


• Doctorate Physical Therapy


• 1 year clinical experience for Home Care

Physical Demands

Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient’s weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
