Job summary

Lehigh Valley Health Network (LVHN) seeks a licensed Physical Therapist for per diem work across multiple locations in the Lehigh Valley. The role focuses on evaluating and treating patients to restore function and independence, using evidence-based techniques and patient education.

Travel to various sites and flexible scheduling are involved. Minimum qualifications include a Bachelor’s degree in Physical Therapy, PA licensure, and BLS.

Qualifications

• Bachelor’s Degree in Physical Therapy required.
• Excellent clinical reasoning to synthesize data from evaluations.
• Ability to travel to patient care assignments - Home Care.
• Ability to work at various locations per diem.
• Current licensure as a Physical Therapist in Pennsylvania Upon Hire; CPR/BLS certification required.

Responsibilities

• Establish and implement PT plans of care to restore function.
• Use evidence-based methods: therapeutic exercises, manual techniques, mobility training, ADL training, modalities.
• Reassess patient progress and adjust plans as needed.
• Communicate patient progress to supervisor, physician(s), and team.
• Provide supervision in accordance with regulatory requirements as appropriate.
• Develop measurable goals and time frames for therapy.
• Ensure cost-effective care and complete required documentation.

Provides assessment of patient condition based on diagnosis from the referring physician. Develops and updates an appropriate plan of care in collaboration with the patient/family to assist in returning them to their prior level of function. Improves or facilitates quality of life through prescribed exercise, hands-on care, and patient education. Helps people achieve fitness goals, regain or maintain their independence, and lead active lives. **Job Duties*** Establishes and implements appropriate physical therapy plans of care to restore function, prevent disability, and help patients reach their maximum performance level.* Implements an individualized treatment plan through the use of skilled, evidence-based therapy techniques such as therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities.* Provides physical therapy treatment consistent with the patients' needs assessment.* Performs reassessment when indicated and modifies physical therapy plans accordingly.* Communicates patient progress and physical limitations to supervisor, physician(s), and other team members.* Implements a physical therapy plan of care to individuals based upon age-specific and population specific needs utilizing a developmental and patient/family directed approach.* Provides supervision in accordance with regulatory requirements, as appropriate.* Establishes a physical therapy treatment plan with measurable goals and time frames.* Provides cost effective care in accordance with departmental billing guidelines.* Completes and submits all documentation based on policies/standards and regulatory requirements.**Minimum Qualifications*** Bachelor’s Degree Physical Therapy* Ability to work at various locations per diem.* Knowledge of best practices in physical therapy.* Demonstrates sound clinical reasoning in synthesizing data gathered in the evaluation and identifying treatment diagnoses.* Must be able to provide safe, compassionate, and competent physical therapy treatments.* Must be able to communicate with patients, family, and co-workers in a friendly, supportive manner.* Ability to travel to patient care assignments - Home Care* American Heart Association Basic Life Support - State of Pennsylvania Upon Hire* PT - Licensed Physical Therapist - State of Pennsylvania Upon Hire**Preferred Qualifications*** Master’s Degree Physical Therapy or* Doctorate Physical Therapy* 1 year clinical experience for Home Care**Physical Demands** Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. \\*Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
