Job description

A Per Diem Physical Therapist is a healthcare professional who provides therapy services on an as-needed basis, typically filling in for staff shortages or covering vacations and leaves of absence. The Physical Therapist is a forward-thinking individual who excels at assessing patients’ strengths and weaknesses and devising creative but effective treatment plans. Therapists must practice discretion & maintain patient confidentiality. Must operate in a high-energy, positive environment and offer patients encouragement to achieve their goals. You will utilize your training and skills to aid patients in healing, recovery, pain management, and maximizing independence. The Staff Physical Therapist examines patients using clinical knowledge, implements treatment plans, and executes the plan of care daily.

What You’ll Need:

• Bachelor’s or Master’s Degree required; Doctorate preferred
• Licensed Physical Therapist in practicing state
• Completed state CEU requirements (NY, NJ)
• Experience with EMR software preferred
• Excellent communication skills
• Strong clinical decision-making skills
• Dedicated work ethic and patient success focus
• Basic computer skills
• Ability to work independently and as part of a team
• Professional demeanor at all times

What You’ll Do:

• Record patient care data according to guidelines, including evaluations, notes, reassessments, discharge plans, and communication
• Handle billing and maintain productivity standards
• Prepare for evaluations by gathering patient history and understanding diagnoses
• Select and administer appropriate tests and measures
• Formulate diagnoses, goals, prognosis, and care plans based on evaluation results
• Establish and modify treatment plans collaboratively with the team and patient
• Refer patients to other professionals as needed and complete discharge plans
• Educate and train patients and support systems
• Perform functional training including ADLs/IADLs, transfers, mobility, and equipment training
• Manage treatment plans and exercise prescriptions for caseload
• Provide safe and effective interventions in compliance with JAG standards
• Document treatments using Prompt EMR
• Attend annual billing and coding seminars
• Maintain respectful interactions with all
