Job summary

VNS Health in New York is seeking a licensed Physical Therapist for per-visit home-based rehabilitation. Conduct assessments, develop individualized treatment plans, and deliver therapy in patients\' homes or care facilities.

You will use clinical technology, including AI-powered documentation, to keep care focused on patient goals while documenting progress and adjusting plans as needs evolve. NY licensure is required; a valid driver\'s license is needed for areas outside Manhattan.

Qualifications

• Minimum of one-year physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern.
• Licensure to practice as a Physical Therapist in New York State.
• Valid driver's license required for all areas outside the borough of Manhattan.

Pay Range per Visit:

If you are applying for the per diem, per visit version of this position, the standard pay range is as follows per visit.

• All areas except the Bronx and the following parts of Brooklyn (Downtown Brooklyn, Williamsburg, and Greenpoint): $70-$75 per visit
• Bronx: Weekday visits - $95 | Weekend visits - $100
• Brooklyn (Downtown Brooklyn, Williamsburg, and Greenpoint only): Weekday visits - $95 | Weekend visits - $100

Pay Range

USD $45.09 - USD $56.39 /Hr.
