Evaluates, plans, and implements goal-oriented Physical Therapy treatment programs focused on restoring function and preventing disability due to trauma, disease, surgery, or other conditions.

Responsibilities

• Participates on committees as requested.
• Delivers high-quality customer service to residents, families, and visitors.
• Models and maintains a safe environment, employing correct body mechanics and appropriate treatment techniques.
• Develops discharge plans, completes summaries, and updates goals throughout treatment.
• Assesses need for DME, ensures proper fitting, and educates residents and families.
• Prepares documentation such as progress notes, re-evaluations, and discharge summaries timely and accurately.
• Gathers evaluation data, including testing and consultations, respecting departmental policies.
• Develops rapport with residents and team members, providing education and fostering goal achievement.
• Implements individualized treatment plans, develops goals, and manages assistive devices and equipment.
• Participates in discharge planning and ensures proper utilization of devices.
• Handles special projects and urgent assignments as needed.
• Adheres to organizational competencies and standards of behavior.

Qualifications

• Graduate of an APTA-accredited program or equivalent.
• Physical ability to perform required tasks, including lifting and moving heavy objects.
• Excellent communication skills and proficiency with computers.

Licenses and Certifications

• Valid Physical Therapist License.
• AHA Basic Life Support (BLS) Certification.
