Qualifications

• Requires Bachelor’s, Master’s, or Doctorate in Physical Therapy.
• Licensed Physical Therapist with BLS certification.
• Strong communication, customer service, teamwork and basic computer skills.

Responsibilities

• Develops and revises treatment plans with patient and family input and provides education and discharge instructions.
• Collaborates with physicians to set goals and discharge plans; evaluates progress.
• Documents evaluations, treatments, and education per protocols.
• Maintains ethical standards, patient confidentiality, and infection prevention practices.
• Communicates with team, participates in meetings, and manages workload.
• Engages in professional development and mentors new staff.
