Job summary

You will conduct initial and discharge assessments, develop treatment plans, and document progress in the EMR, while collaborating with interdisciplinary teams and educating patients, families, and staff.

Responsibilities include mentoring interns and coordinating equipment, vendor contacts, and communications with external physicians.

Qualifications

• Bachelor's degree in Physical Therapy is required; doctoral degree preferred.
• GA Physical Therapy license or eligibility; BLS certification required.
• Minimum one year of rehab, SNF, LTACH, or acute care experience preferred.
• Basic computer literacy.

Responsibilities

• Perform evaluations, set goals, and create treatment plans aligned with patient needs.
• Deliver therapy sessions and patient education; document progress in EMR.
• Participate in hospital and discipline-specific skills assessments.
• Mentor interns and team members in clinical and professional development.
