Job summary

Aultman Health Foundation is seeking a Physical Therapist to interpret referrals, evaluate, adjust, and discharge patient treatment per Ohio rules. You will develop plans of care, document thoroughly, and collaborate with an interdisciplinary team in a patient-centered setting.

Responsibilities include maintaining competency, ensuring quality improvement, and communicating effectively with patients and staff while upholding HIPAA standards and organizational values.

Qualifications

• Graduation from a CAPTE accredited Physical Therapy program.
• Current PT license from the State of Ohio OT, PT, and AT Board.
• Current BLS certification is required.
• Basic computer skils are needed.

Responsibilities

• Develops patient plans of care and ensures adherence.
• Keeps the UD informed of significant problems or concerns.
• Responds to patient/family complaints.
• Orients new personnel to charts, forms and documentation requirements.
• Provides peer review of job performance of co-workers on a regular basis.
• Assists with quality improvement processes as needed or directed.
• Supports compliance with federal, state and local rules and regulations.
• Communicates professionally with patients, families and staff.
• Demonstrates flexibility in meeting program needs and workflow changes.
• Promotes teamwork and collaboration with the interdisciplinary team.
• Maintains infection control and safety policies.
• Completes annual competency requirements.
• Communicates requests and concerns to appropriate personnel.
• Maintains confidentiality per HIPAA and organizational standards.
• Supports organizational goals and Code of Conduct.
