Job summary

OhioHealth in Ashland is seeking a Physical Therapist to assess and treat patients, develop plans of care, and coordinate discharge planning within a multidisciplinary team.

You will supervise PTAs, educate patients and families, and ensure accurate documentation in the medical record. A valid Ohio PT license and driver’s license are required, with daily patient care in a home care setting.

Qualifications

• Bachelor's degree in Physical Therapy is required.
• Licensed to practice in Ohio as a PT.
• For Home Care, valid driver’s license and vehicle record may be required.
• Must be at least 18 years old with acceptable MVR and proof of insurance, if applicable.

Responsibilities

• Direct patient care 90-100% following assessment and plan of care.
• Evaluate patient functional status and implement treatment.
• Collaborate with multidisciplinary team for discharge planning.
• Supervise PTAs and rehabilitation techs.
• Document evaluations, plan of care, progress, and discharge details.

Based on problem(s) identified, the physical therapist establishes and carries out a plan of care to include patient treatment and reassessment, patient/family instruction, discharge planning, documentation in the medical record and providers and other medical staff. Oversees and supervises provision of care by physical therapist assistants.Responsibilities And Duties:Direct Patient Care 90-100%Evaluates and treats patients:After provider referral, evaluates patient physical abilities and functional status, develops and implements plan of care based on evaluation findings and appropriate treatment techniques and initiates patient treatment.Carries out plan of care with reassessment and changes to plan of care as appropriate.Responsible for maintaining/meeting the productivity expectations of their department.Oversees and supervises provision of care by physical therapist assistants.Plans and helps coordinate discharge plan in collaboration with the multidisciplinary team:Instructs patient and care giver/family.Recommends DME required for discharge.Provides information to providers, nurses, case managers, social workers, and other team members.Involved in the multidisciplinary patient plan of care and discharge readiness plan.Documents evaluation, plan of care, progress toward goals, discharge plan/continuity of care for multidisciplinary team, and other involved medical and insurance parties.Indirect Patient Care 1-10%Department Functions:Supervises patient assignments and duties of the PTAs and rehabilitation tech(s).Is involved in department quality improvement through data collection and participation in team and staff meetings.Assumes duties delegated by management in the absence of department management.Education/Mentoring:Takes an active role in orienting new staff members.Serves as clinical instructor for affiliating PT and PTA students.Serves as an advocate for discipline on the multidisciplinary team.Represents department at problem-solving and committee meetings.The major duties, responsibilities and essential functions listed above are not intended to be all-inclusive of the duties, responsibilities, and essential functions to be performed by employees in this job.
