Benedictine Living in Minnesota is seeking a dedicated Physical Therapist to evaluate residents, perform ongoing assessments, and implement therapy interventions. You will develop and revise plans of care, set functional goals, and coordinate discharge planning to meet both short- and long-term resident needs.

The Physical Therapist is responsible and accountable for residents’ evaluations, ongoing assessments, and the planning and implementation of therapy interventions.
Develops and revises plan of care based on the residents’ needs and establishes functional goals, outcomes, and discharge plans. Plans for residents’ discharge and anticipates for long term and/or short term residents’ needs. Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents.
