Holland Home's Care Resources division seeks a part-time Physical Therapist to join our Lowell, MI team. This 25-hour weekly role operates Monday through Friday with occasional travel to participant homes in Kent County.

The position focuses on delivering comprehensive PT services within our multi-disciplinary clinic and day center for the frail elderly. Responsibilities include intake screenings, evaluations, care planning, therapy, staff training, and timely documentation.

Qualifications

  • Must have a Bachelor's Degree in Physical Therapy from an accredited program.
  • Current Michigan Physical Therapist license.
  • Valid Michigan driver's license and reliable transportation for home visits.

Description

Physical Therapist | Part-time, Lowell

Care Resources is seeking a part-time Physical Therapist to join our fabulous team of health care professionals! This position is responsible for providing physical therapy services to the participants of Care Resources.

This is a Monday – Friday day shift, hourly position. No weekends and no holidays! It is based at our multi-disciplined clinic and day center in Lowell and will occasionally include home visits for our participants.

Responsibilities
  • Performs intake screenings as assigned and makes appropriate recommendations for services.
  • Completes participant evaluations as needed and develops a plan of care.
  • Provides treatment and implements physical therapy procedures
  • Trains staff and caregivers on follow-through programs
  • Participates in care plan development and discharge planning for participants
  • Assesses the need for adaptive durable medical equipment, specialized shoes, and prosthesis
  • Communicate therapy progress and discharge plans to the team
  • Assists in training and provides professional supervision for PTA(s) and students
  • Completes documentation as required in timely manner
  • Develop special programs according to participant needs
  • Provides education and training to staff, participants, and caregivers

Location: Care Resource PACE of Lowell |11730 Fulton St E, Lowell, MI 49331

This position also involves some travel to client homes in Kent County.

FTE: 25 hours per week

Qualifications:
  • Bachelor's Degree or equivalent from an accredited physical therapy program
  • Physical Therapist license in the state of Michigan
  • Valid Michigan driver's license, auto insurance, and reliable means of transportation for home visits
  • Top candidates will have a minimum of 1 year of experience working with a frail or elderly population

About Care Resources

Care Resources operates the PACE program (Program of All-inclusive Care for the Elderly) in Lowell at 11730 Fulton St. Our services include medical care and social services, prescription drugs, medical equipment, dental, eye and foot care, hospital care, medical transportation, home care, emergency, rehab, and social and recreational activities at the Day Center. To be eligible for the PACE program, seniors must be 55 and older, a Kent County resident, able to live safely at home, and meet the nursing facility level of care criteria.
