Job summary

Senior Care Partners PACE, Kalamazoo area, seeks a Physical Therapist to float between Kalamazoo and Calhoun counties. You will evaluate mobility, develop care plans, and provide therapy using evidence-based methods to help older adults maintain independence and safety.

You’ll collaborate with an IDT, educate families, and mentor staff while supporting transitions home and participation in quality initiatives.

Qualifications

  • DPT required; grandfathered exemptions as applicable.
  • Graduated from an approved physical therapy program.
  • Current Michigan Physical Therapist license required.
  • Minimum 1 year experience with frail/elderly or long-term care populations.
  • Strong communication, organization, problem-solving and collaboration skills.
  • Working knowledge of Microsoft Office.
  • Current AHA CPR certification or willingness to obtain.

Responsibilities

  • Evaluate participants’ mobility, abilities and rehab needs.
  • Develop and implement individualized physical therapy plans of care.
  • Provide therapy via exercise, modalities, manual therapy and evidence-based interventions.
  • Float between Kalamazoo and Calhoun counties per participant/program needs.
  • Collaborate with Interdisciplinary Team to coordinate care.
  • Assess and recommend adaptive and durable medical equipment transitions.
  • Educate participants, families and caregivers to promote safety and independence.
  • Support physical wellness programming and develop innovative programs.
  • Mentor PTAs, rehab tech PCAs and PT students when applicable.
  • Assist participants transitioning home from hospitals or SNFs/assisted living.
  • Participate in quality improvement and strive for exceptional customer service.

Who is P.A.C.E?

The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

Senior Care Partners PACE is seeking a Physical Therapist – Float to provide compassionate, participant-centered care across our Kalamazoo and Calhoun County locations. This is an opportunity to use your clinical expertise in a dynamic setting where you’ll help older adults maintain mobility, independence, safety, and quality of life—both at our PACE centers and in the community.

What You’ll Do
  • Evaluate participants’ mobility, physical abilities, and rehabilitation needs.
  • Develop and implement individualized physical therapy plans of care.
  • Provide therapy through therapeutic exercise, modalities, manual therapy, and other evidence-based interventions.
  • Float between Kalamazoo and Calhoun counties based on participant and program needs.
  • Collaborate closely with the Interdisciplinary Team (IDT) to coordinate participant-centered care.
  • Assess and recommend adaptive and durable medical equipment and support equipment transitions.
  • Educate participants, families, and caregivers to promote safety and independence.
  • Support physical wellness programming and help develop innovative programs for participants.
  • Provide guidance and mentorship to PTAs, rehab tech PCAs, and PT students when applicable.
  • Support participants transitioning home from hospitals, skilled nursing, assisted living, and other care settings.
  • Participate in quality improvement initiatives and promote exceptional customer service.
What You Bring
  • Doctorate in Physical Therapy (DPT), unless grandfathered under applicable requirements.
  • Graduate of an approved physical therapy program.
  • Current, unencumbered Michigan Physical Therapist license.
  • At least 1 year of experience working with frail, elderly, or long-term care populations.
  • Strong communication, organization, problem-solving, and collaboration skills.
  • Working knowledge of Microsoft Office.
  • Current AHA CPR certification or willingness to obtain certification.

Work Environment
  • Has a visible presence while working in the center and is subject to frequent interruptions.
  • Communicates with staff, participants, caregivers, the board of Senior Care Partners PACE and other organizations as necessary.
  • Works beyond normal working hours and on weekends and holidays when necessary.
  • May be subject to exposure and handling of infectious waste, diseases, conditions, or hazardous chemicals, etc., including potential exposure to TB, AIDS, and/or the Hepatitis B virus.
