Job summary

CONSUMER DIRECTED SERVICES INC. in Illinois seeks a licensed Physical Therapist to evaluate and treat patients with orthopedic, neurologic, developmental, vestibular, pelvic health, and functional needs. You will create individualized care plans and modify treatments based on patient response and evidence.

You will perform safe patient transfers, educate patients and families, document findings, and supervise support staff to ensure high-quality care and ethical practice.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Valid licensure as a Physical Therapist.
• Basic Life Support (BLS) certification required.

Role Overview:

The Physical Therapist evaluates and treats patients with diverse rehabilitative needs, including orthopedic, neurological, developmental, vestibular, pelvic health, and functional conditions caused by injury, illness, disease, or age-related changes. They assess patients of all ages, develop individualized care plans, and modify treatments based on patient response and evidence.

Key Responsibilities:

Perform patient transfers safely, demonstrate proper body mechanics, and provide compassionate care. Design and adjust treatment plans aimed at functional improvement and discharge goals. Educate patients, families, and healthcare providers using effective methods. Conduct examinations, select appropriate assessment techniques, and document findings accurately. Supervise support personnel, monitor care quality, and ensure compliance with legal and ethical standards. Complete required continuing education, particularly for specialized patient populations.

Department Duties:

Evaluate impairments related to orthopedic and neurologic conditions, perform objective testing (range of motion, strength, functional tasks), and utilize modalities such as ultrasound or iontophoresis. Apply therapeutic techniques like taping and communicate progress and recommendations with referring providers.

Qualifications & Skills:

Bachelor's Degree in Physical Therapy, valid licensure as a Physical Therapist, and Basic Life Support certification. Ability to perform safe patient handling, demonstrate empathy, and utilize clinical judgment in treatment planning. Strong communication, documentation, and team collaboration skills are essential.
