Job summary

Baystate Health is seeking a Physical Therapist for our outpatient rehab team in Springfield, MA. This full-time role supports patient access with a 40-hour schedule, and a $5,000 sign-on bonus is available for external hires.

Qualifications

• Bachelor’s degree in Physical Therapy is required; Doctorate preferred.
• Massachusetts PT licensure is required to practice in MA.
• BLS/CPR certification for healthcare providers is required.

Responsibilities

• Evaluates patients, develops treatment plans, and assists in discharge planning within 24 hours.
• Applies and modifies treatment per plan; trains patients and family on procedures and equipment.
• Documents initial consults, progress notes, and discharge reports in a timely manner.
• Provides information and counseling to patients, families, and care teams.
• Manages scheduling, billing, equipment usage, and caseload coverage per dept. policy.
• Participates in on-the-job orientation and intern training; contributes to intern evaluations.
• Demonstrates clinical knowledge and sets professional development objectives.
• Meets productivity expectations as directed by supervisor; engages in in-service training.
• Attends external seminars and presents in-service training as required.
