Job summary

Baystate Franklin Medical Center, Greenfield, MA, is seeking a full-time Physical Therapist to join the outpatient rehab team. The role involves evaluating patients, designing individualized plans, and supporting discharge planning in a collaborative setting.

Qualified candidates include those with a Bachelor’s in Physical Therapy (Doctorate preferred), MA licensure, and BLS CPR certification. Both experienced therapists and new grads are encouraged to apply, with a competitive benefits package

Qualifications

• Bachelor’s degree in Physical Therapy (Doctorate preferred).
• Massachusetts Physical Therapist Licensure.

• American Heart Association BLS CPR.

Responsibilities

• Evaluates patients, develops and applies treatment plans per department guidelines.
• Assists discharge planning and coordinates with rehab team for patient goals.
• Documents initial consult, progress notes, and discharge summaries within required timelines.
• Provides patient and family counseling to enhance understanding of conditions and RX goals.
• Insures appropriate use of equipment and adheres to safety and privacy policies.
• Participates in intern training and contributes to staff development.
• Maintains productivity and participates in ongoing in-service training.
• Contributes to care plan reviews and interdisciplinary communication.
