Job summary

ProCare Rehabilitation LLC is seeking a Physical Therapist to deliver evidence-based, hands-on care in an outpatient setting across orthopedic, neurological, and vestibular conditions. You will perform evaluations, design and adjust treatment plans, monitor progress, and maintain compliant documentation.

The role emphasizes patient and family education, coordination with referring clinicians, and participation in clinic meetings to support a collaborative, patient-first environment.

Qualifications

• Expertise in Physical Therapy and Rehabilitation, including evaluation and treatment planning.
• Strong patient care, empathy, communication, and individualized treatment; home care/community-based therapy beneficial.
• Foundation in medicine related to musculoskeletal, neurological, and vestibular conditions; interpret findings and collaborate with providers.
• DPT or equivalent and active state licensure (or eligibility).
• Experience in outpatient orthopedics; exposure to neurological and vestibular rehabilitation is preferred.
• Documentation skills and familiarity with EMR; adhere to professional, ethical, and regulatory standards.
• Ability to work on-site in a team-based environment, manage a varied caseload, and maintain professional communication with patients and colleagues.

Responsibilities
