Job summary

ProCare Rehabilitation L.L.C. is a clinician-led outpatient physical therapy practice delivering personalized care.

The Physical Therapist will perform comprehensive evaluations across orthopedic, neurological, and vestibular conditions and develop individualized treatment plans. Responsibilities include tracking patient progress, updating documentation, educating patients and families, and coordinating with referring clinicians.

Qualifications

• DPT or equivalent degree; active state licensure or eligibility.
• Experience in outpatient orthopedics; neurological/vestibular exposure preferred.
• Strong documentation and EMR familiarity.

ProCare Rehabilitation LLC is a physical therapist owned-and-operated practice founded in 1998 in Flemington, NJ, and has since grown to multiple locations serving their local communities. Each clinic is known as a trusted, neighborhood physical therapy provider with a strong reputation for personalized, "mom-and-pop" style care. The practice began with a focus on orthopedic rehabilitation, including spine and extremity injuries, as well as postoperative care. Over time, ProCare expanded services to include treatment for neurological disorders and vestibular rehabilitation, responding to changing community needs. In a healthcare landscape dominated by large hospital systems and corporations, ProCare continues to thrive by prioritizing quality, individualized service and patient-centered care.

Role Description:

The Physical Therapist will provide evidence-based, hands-on care to patients across orthopedic, neurological, and vestibular conditions in an outpatient setting. Day-to-day responsibilities include performing comprehensive evaluations, developing and implementing individualized treatment plans, monitoring progress, and updating documentation in accordance with clinical and regulatory standards. The role also involves patient and family education, coordination of care with referring clinicians and other healthcare professionals, and participation in clinic meetings and quality improvement initiatives. This is a full-time, on-site role requiring consistent in-person interaction with patients and team members. The Physical Therapist will contribute to a collaborative, patient-first environment that supports recovery, functional independence, and long-term wellness.

Qualifications:

• Demonstrated expertise in Physical Therapy and Rehabilitation, including evaluation, treatment planning, and therapeutic interventions.
• Strong Patient Care skills, with a focus on empathy, communication, and individualized treatment; experience with Home Care or community-based therapy is beneficial.
• Solid foundation in Medicine as it relates to musculoskeletal, neurological, and vestibular conditions, with the ability to interpret clinical findings and collaborate with other providers.
• Current Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program and active state licensure (or eligibility) as a Physical Therapist.
• Experience in outpatient orthopedics; exposure to neurological and vestibular rehabilitation is preferred.
• Strong documentation skills and familiarity with electronic medical records, along with adherence to professional, ethical, and regulatory standards.
• Ability to work effectively in an on-site, team-based environment, manage a varied caseload, and maintain professional communication with patients and colleagues.
