Physical Therapist - Outpatient Program Full-Time (Healthcare)

Westminster Oaks, a beautiful campus surrounded by sprawling oak trees in Tallahassee, FL, is seeking a full-time Physical Therapist for our Outpatient program. The Physical Therapist will conduct patient/resident assessments, develop care plans, provide direct treatment, and consult on a general rehab caseload. The role involves participation in the clinical team process, teaching and training caregiver staff, clinical program development, and supervising licensed therapists, assistants, technicians, aides, and students as required.

ESSENTIAL JOB DUTIES:

1. Evaluate and treat physical conditions to restore functions and improve mobility as indicated and ordered by MD. Design and implement Care Plans.
2. Conduct assessments and screenings to determine therapy needs. Provide treatments including electrical stimulation, hot/cold packs, and therapeutic massage.
3. Maintain timely and accurate documentation, including updating information in the system.
4. Perform resident home assessments as part of the discharge process.
5. Screen residents during admission and conduct quarterly and annual screenings to assess therapy needs.
6. Familiarity with Casamba/Rehab Optima software is preferred.

QUALIFICATIONS:

1. Must be currently licensed in Florida as a Physical Therapist.
2. One year of long-term care experience is desirable.
3. Experience in med-surg and home health is helpful.
4. Experience working with older adults is preferred.
