Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients of all ages, in inpatient and outpatient settings. The role emphasizes safety, evidence-based practice, and collaborative care.

Qualifications

• Licensed Physical Therapist (PT) in Illinois
• Basic Life Support (BLS) within 30 days
• Bachelor's Degree in Physical Therapy

Responsibilities

• Performs patient transfers with assistance as needed.
• Designs an appropriate plan of care linking goals, treatment, outcomes and discharge.
• Educates patients and caregivers using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Completes documentation in EMR and supports billing.
• Communicates progress with referring providers.
