Qualifications

• DL-HC license required only if assigned to Home Care department.

Responsabilités

• Direct patient care after physician referral (approx. 90%).
• Administer portions of the assessment as directed by the PT.
• Assist in reassessment and report patient progress.
• Help implement and adjust plans of care.
• Attend multidisciplinary care meetings.
• Coordinate discharge plans with the team.
• Educate patients and families/caregivers.
• Identify equipment needs for discharge.
• Communicate with physicians, nurses, care managers and staff.
• Document evaluations and progress with co-signature by supervising PT.
• Support indirect department functions and process improvements.
• Delegate tasks to Rehab Technicians.
• Contribute to QA and utilization review.

Formation

Bachelor's Degree: Physical Therapy

Job Description Summary

To competently interpret the patients history and baseline level of function and then to complete an assessment appropriate to the patients areas of deficits. Based on problems identified, the physical therapist establishes and carries out a plan of care to include patient treatment and reassessment, patient/family instruction, discharge planning, documentation in the medical record and communication with physicians and other medical staff. This position helps patients achieve maximum functional outcomes. Actively participates in departmental functions.

Responsibilities And Duties

Driving is an essential function of the job.

• 90% - Direct Patient Care – Evaluates and treats patients after physician referral.
• Administers portions of the assessment that is assigned by the Physical Therapist (PT) in which Physical Therapy Assistant (PTA) has demonstrated to be competent.
• Assists in reassessment process, reporting patient performance to the PT.
• Assists PT in implementation of the patients plan of care based on occupational performance frames of reference(s): bio-mechanical, rehabilitation, sensorimotor.
• Attends multidisciplinary patient care meetings.
• Plans and helps coordinate discharge plan with multidisciplinary team.
• Educates patient and caregiver/family.
• Recommends Medical Equipment required for discharge.
• Provides information to Physicians, Nurses, Care Managers, Social Workers, and other team members.
• 5%.
• Documents evaluation performance directed and interpreted by the PT, progress towards goals, discharge plan/continuity of care for the multidisciplinary team, and other involved medical and insurance parties.
• All documentation to be co-signed by the supervising PT.
• 5% - Indirect Department Functions.
• Delegates assignments to Rehab Technicians.
• Remains involved in department process improvements utilization review and quality assurance program.

Educates Others About Physical Therapy

• Takes active role in orienting new staff members.
• Assists in clinical instruction of PTA students.
• Educates other disciplines and medical/hospital personnel on PT purpose, goals and treatment policies/procedures.
• Assumes other duties delegated by management.
