Job summary

Stanford Health Care Tri-Valley is seeking a licensed Physical Therapist for an outpatient hospital setting. You will assess patients, create and implement individualized treatment plans, and document progress in the medical record.

Collaboration with physicians and the care team is essential, with supervision of PTA and aides as needed. Required are a CA PT license and AHA BLS certification, plus 2–3 years of PT experience.

Qualifications

  • Bachelor's degree in physical therapy from an accredited university required.
  • 2–3 years of experience preferred.
  • Current California PT license and American Heart Association BLS certification required.
  • Knowledge of Medicare documentation standards and hospital safety/JCAHO expectations.

Responsibilities

  • Direct patient care, assessing and treating patients in outpatient hospital settings.
  • Develop and carry out individually designed treatment programs.
  • Document evaluations, plans, progress notes, and discharge summaries.
  • Coordinate with physicians and care team; supervise PT aides and students as needed.
  • Maintain patient education and assistive device fitting where appropriate.

Provides appropriate treatment of patients using specific modalities and treatment techniques. Accurately documents treatments and care of patients and communicates with other team members. Participates in individual and department continuous improvement and education activities.LocationsStanford Health Care Tri-ValleyWhat you will doDirect patient care, including the assessment and treatment of patients, developing and carrying out individually designed treatment programs.Performs and documents an initial evaluation, interprets data to identify problems, and determines functional limitations prior to intervention.Performs ongoing documentation in the patient medical record with functional limitations, goals and appropriate timelinesCommunicate with referring Physician to obtain certification of plan of care and document as appropriate.Includes the patient/caregiver in treatment process, evaluation of effectiveness and discharge as necessary.Evaluates the effectiveness of treatment at various stages and makes adjustments to achieve maximum benefit or discharges when treatment no long neededEnsures patient education, proper fit and use for assistive devices as necessary.Supervises PTA, PSM Aide, students and volunteers as the clinical schedule dictatesParticipates in team and hospital wide meetings, committees and initiatives as assignedAssists with department finances with accurate daily billing practices, active daily maintenance of patient care schedule.Employee must perform all duties and responsibilities in accordance with the C-I-CARE Standards of the Hospital.
