Provides appropriate treatment of patients using specific modalities and treatment techniques. Accurately documents treatments and care of patients and communicates with other team members. May be asked to perform other duties as assigned.Direct patient care, including the assessment and treatment of patients, developing and carrying out individually designed treatment programs.Performs and documents an initial evaluation, interprets data to identify problems, and determines functional limitations prior to intervention.Performs ongoing documentation in the patient medical record with functional limitations, goals and appropriate timelinesCommunicate with referring Physician to obtain certification of plan of care and document as appropriate.Includes the patient/caregiver in treatment process, evaluation of effectiveness and discharge as necessary.Evaluates the effectiveness of treatment at various stages and makes adjustments to achieve maximum benefit or discharges when treatment no long neededEnsures patient education, proper fit and use for assistive devices as necessary.Supervises PTA, PSM Aide, students and volunteers as the clinical schedule dictatesParticipates in team and hospital wide meetings, committees and initiatives as assignedAssists with department finances with accurate daily billing practices, active daily maintenance of patient care schedule.Employee must perform all duties and responsibilities in accordance with the C-I-CARE Standards of the Hospital.
