Job summary

Carle Health is seeking a Physical Therapist to create and modify orthotics, evaluate patients of all ages, and adjust plans of care. The role emphasizes education, documentation, and collaboration with providers. Eligible for a sign-on bonus and potential relocation assistance.

New graduates and experienced PTs welcome; licensure requirements apply. This position supports patient care across multiple sites within Carle Health.

Qualifications

• Licensed Physical Therapist (PT) per IDFPR requirements.
• BLS certification within 30 days (AHA).
• Complete required continuing education on acute stroke care.

Responsibilities

• Designs and adjusts plan of care incorporating goals, treatment, outcomes and discharge plan.
• Educates patients, families, and caregivers with effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes of care.
• Performs examinations with valid methods relevant to chief complaint and history.
• Documents according to CTSCP67 policy.
• Screens patients to determine need for further therapy services and monitors care by staff.
• Complete annual education for DNV Hip and Knee programs if applicable.
• Evaluates impairments related to orthopedic and neurologic conditions.
