You will evaluate, develop, and implement personalized plans of care, monitor progress, and educate patients and caregivers on exercises, safety, and home programs.

Qualifications

• Active New Jersey Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Experience in home health or related settings preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Complete comprehensive physical therapy evaluations in the home setting.
• Develop personalized plans of care based on patient needs, functional limitations, and rehabilitation goals.
• Provide skilled physical therapy interventions to improve mobility, strength, balance, coordination, and overall function.
• Monitor patient progress and adjust treatment recommendations when appropriate.
• Educate patients and caregivers on exercises, mobility techniques, safety strategies, and home programs.
• Complete electronic documentation accurately and within required timeframes.
• Communicate with physicians, caregivers, office staff, and other members of the care team as needed.
• Maintain compliance with company policies, professional standards, and applicable state requirements.

Position Overview

Homeside Rehab is expanding our therapy team in Piscataway, New Jersey and is currently seeking a licensed Physical Therapist to provide one-on-one rehabilitation services in patients’ homes.

This opportunity is ideal for a PT who values independence, flexible scheduling, and the ability to build meaningful relationships with patients and families. You will work closely with our clinical and administrative teams while managing a caseload within your assigned service area.

What You’ll Do

• Complete comprehensive physical therapy evaluations in the home setting
• Develop personalized plans of care based on patient needs, functional limitations, and rehabilitation goals
• Provide skilled physical therapy interventions to improve mobility, strength, balance, coordination, and overall function

Position Details

Location: Piscataway, NJ 08854
Pay: $65 - $80 per hour
Employment Options: Full-Time, Part-Time, or Per Diem

Scheduling may include:
