Job summary

Jethro Health System in Massachusetts is seeking a Part-Time or Per Diem Physical Therapist (PT) or Physical Therapist Assistant (PTA) to provide skilled home health visits in the Lynn area. The role focuses on maintaining patient independence and quality of life in the comfort of patients' homes.

The position requires an accredited PT program degree, MA licensure for PTs, CPR certification, and a driver’s license.

Qualifications

  • Associate's degree and graduation from an accredited Physical Therapy program.
  • One year of physical therapy experience preferred.
  • CPR Certification
  • Possess valid driver's license

Responsibilities

  • Provides care and services in accordance with the plan of care.
  • Assists community members and health field personnel to assess, plan for, and provide needed health and related services.
  • Documents care provided in an accurate and timely manner.
  • Provides teaching to clients and families.
  • Coordinates and collaborates with other members of the health care team, community agencies, and referral sources.
  • Communicates the plan of care with patients and families and insures understanding of the exercise program prescribed.
  • Responsive to meeting or exceeding patient/client expectations and promoting a positive image of the company
  • Adheres to HIPAA laws, maintains patient confidentiality, and adheres to all agency policies and procedures.
  • Attends continuing education programs.
  • Other duties assigned by the department manager.

Responsibilities


  • Provides care and services in accordance with the plan of care.

Requirements/Qualifications
