Job summary

Qualifications

• Graduation from an accredited physical therapy program.
• Active or pending state license to practice PT.
• Interest in working with the senior population.
• Superior customer service and interpersonal communication skills.
• Strong communication and interpersonal skills to work effectively with patients, families, and colleagues.

Responsibilities

• Evaluate patients to assess strength, balance, mobility, and functional impairments.
• Develop and implement personalized treatment plans that improve mobility and function.
• Deliver care at one or more assisted living locations.
• Provide care with independence in assisted living communities with team support.
• Educate patients and families on home exercise programs and strategies for maintaining independence and quality of life.
• Document patient care services in EMR in compliance with state and facility standards.
• Engage in educational programs, in-service meetings, and workshops for professional growth.

As a Physical Ther apist, you will:

• Evaluate patients to assess strength, balance, mobility, and functional impairments
• Develop and implement personalized treatment plans that focus on improving mobility, reducing pain, and enhancing overall function
• Deliver care at one or more assisted living locations, providing meaningful services to a diverse group of seniors
• Provide care with a high level of independence in assisted living communities, while always supported by the broader Engage team for guidance and resources
• Educate patients and families on home exercise programs and strategies for maintaining functional independence and a better quality of life
• Document patient care services in compliance with state and facility standards using EMR
• Enhance professional growth and development through participation in educational programs, in-service meetings, and workshops
