Job summary

Fusion Physical Therapy Partners, a network of New England private practices, seeks a Physical Therapist to serve seniors at assisted living facilities in Salem, NH. You will help geriatric patients regain independence while joining a collaborative Engage Senior Therapy team.

You’ll evaluate patients, create personalized plans, and deliver interventions across locations, documenting care in EMR and advancing professional growth within a supportive environment.

Qualifications

• Graduation from an accredited physical therapy program.
• Active state license to practice PT.
• Interest in working with the senior population and helping them regain independence.
• Superior customer service and interpersonal communication skills.
• Strong communication and interpersonal skills to work with patients, families, and colleagues.

Responsibilities

• Evaluate patients to assess strength, balance, mobility, and functional impairments.
• Develop and implement personalized treatment plans focusing on mobility and pain reduction.
• Deliver care at one or more assisted living locations for seniors.
• Provide therapeutic interventions such as manual therapy and therapeutic exercises.
• Promote safety and fall prevention.
• Document patient care in EMR in compliance with standards.
• Assist in professional growth through education and workshops.
• Build bonds with patients, families, and healthcare teams.

As a Physical Therapist, you will:

• Evaluate patients to assess strength, balance, mobility, and functional impairments.
• Develop and implement personalized treatment plans that focus on improving mobility, reducing pain, and enhancing overall function.
• Deliver care at one or more assisted living locations, providing meaningful services to a diverse group of seniors.
• Provide therapeutic interventions such as manual therapy, therapeutic exercises, gait training, and balance programs.
• Promote safety and fall prevention by identifying risks and implementing preventive measures.
• Provide care with a high level of independence in assisted living communities, while always supported by the broader Engage team for guidance and resources.
• Educate patients and families on home exercise programs and strategies for maintaining functional independence.
• Document patient care services in compliance with state and facility standards using EMR.
• Enhance professional growth and development through participation in educational programs, in-service meetings, and workshops.
• Develop bonds not just with patients, but with their families, doctors, and fellow team members, ensuring collaborative care and successful outcomes.
