Source Code in Modesto, California is seeking a Physical Therapist to join our IDT team. You will focus on care planning, coordinate and deliver restorative therapies, and work to maximize independence and safety for participants.

This role offers lower patient volumes, opportunities to build long-term relationships, collaboration with other therapists and providers, and a balanced schedule with no after-hours documentation.

Qualifications

Responsibilities

• Conduct Physical Therapy assessments in facility and home settings to define care needs and goals.
• Participate in IDT meetings to develop participant care plans.
• Deliver and document interventions per care plans.
• Provide active and passive exercises, gait training, and ADL therapy.
• Supervise Therapy Assistants and provide training for Functional Maintenance Programs.
• Educate staff, participants, and families on therapeutic equipment use and care.

Full-Time

The Physical Therapist is a key member of the IDT, focusing on care planning and the coordination and delivery of restorative and rehabilitative physical therapy services to participants, working to maximize independence and safety.

This role is different because physical therapists at our facility:

• Lower patient volumes — no more seeing 3-4 patients an hour
• Form long-term relationships with participants rather than short-term rehab stints
• Collaborate with other therapists, providers, and disciplines
• Have work/life balance – no after-hours documentation

On the day-to-day, you will:

• Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals
• Actively participate in IDT meetings to develop participant care plans
• Deliver and document interventions as agreed upon in participant care plans
• Provide active and passive exercise, muscle re-education, gait, and functional training of ADLs and treatments including massage, heat, sound, water, light, or electricity
• Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)
• Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment

Must-Haves:

• Bachelor’s degree in physical therapy; master’s or doctoral degree preferred
• Valid California Physical Therapy License, or proof of license eligibility
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
