Job summary

Kadima Health and Nursing at Lakeside is seeking a Physical Therapist to join our team, providing therapeutic services to residents in a skilled nursing/long-term care setting. You will evaluate and treat residents, follow a plan of care, document outcomes, and educate patients and families.

Current PA license is required; SNF/long-term care experience preferred. Competitive pay rates and a supportive environment accompany this role.

Qualifications

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Responsibilities

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Assured all treatment is delivered in accordance with an established plan of care.
• Provide clinical support and instruct patients, families, and caregivers.
• Measures & records patient’s motor function, strength, and muscle performance.
• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary.
• Completes documentation in accordance with company policy and state/federal regulations.
