Job Summary: ThePhysical Therapist shall screen, evaluate, re-evaluate, discharge plan, andmake revisions in any established plan of care as supervised by the physician.

Qualifications:

• Bachelor's,Master’s, or Doctorate in physical therapy from an accredited program.
• Certificationand licensure requirements as established by state and national boardassociations.
• Activestate license required.
• Excellentorganizational and time management skills
• Excellentwritten and verbal communication skills
• Possesscurrent working knowledge of Functional Pathways policies and procedures,and clinical programs with application skills to therapy disciplines.
• Ability tosupervise Physical Therapy Assistants
• Completionof all required screenings, certifications, and licensure. Screeningrequirements may vary by location.
