Job summary

University Hospitals Pain Management seeks a licensed Physical Therapist to lead rehabilitation within the care continuum. You will evaluate patients and develop individualized treatment plans to restore function and promote health.

You will deliver evidence-based therapies, educate patients and families, coordinate discharge planning, and support department operations while safeguarding patient PHI and adhering to UH policies.

• Delivers effective, evidence-based treatment techniques.
• Educates patients, families, and caregivers on health information and rehabilitation.
• Initiates and participates in discharge planning.

• Performs effective treatment techniques in a competent manner.
• Educates patient/family/caregiver in applicable health information, treatment and prevention.
• Initiates and participates in the discharge planning process.
• Performs other department activities such as, but not limited to documentation, billing, support of department operations, etc.
• Performs other duties as assigned.
• Complies with all policies and standards.

Job description

A Brief Overview

Organizes and conducts Physical Therapy program to facilitate the rehabilitation of people with physical and/or psychosocial disabilities. The Physical Therapist directs the patient's participation in selected activities/treatments to restore, re-enforce and correct pathology to promote and/or maintain good health.
