Job summary

Safe Life Home Health Care, LLC is seeking a licensed Physical Therapist to provide PT services to patients in the Oglesby area and surrounding counties. You will supervise PT Assistants, coordinate care with physicians, and support families with home therapy programs.

The role requires travel to patient residences, ability to evaluate and update plans of care within required timeframes, and an emphasis on quality, safety, and documentation.

Qualifications

• Must be licensed Physical Therapist in Illinois.
• Minimum of 1 year home health care clinical experience.
• Must have a car with proof of insurance and reliable transportation.

Responsibilities

• Assists physician in evaluating patients’ level of function and setting treatment goals.
• Performs initial evaluations within 48 hours of referral and re-evaluations every 30 days.
• Prepares clinical and progress notes and required summaries.
• Revises plan of treatment and communicates changes to physician.
• Advises and consults with patient’s family and other health care staff.
• Coordinates care with the DON and other staff.
• Monitors patient status and reports changes to schedule or condition.
• Supervises the Physical Therapy Assistant.
• Instructs patient/family in home therapy and equipment use.
• Communicates with patients, caregivers, and agency staff clearly.

Position Overview

Provides Physical Therapy services to patients as directed by the plan of care in line with the philosophy, objectives, and policies of the agency. Supervises Physical Therapy Assistants and maintains responsibility for the implementation and follow-through of PT procedures.

Responsibilities

• Assists the physician in evaluating patients’ level of function, establishing treatment plan and goals.
• Performs initial evaluations within 48 hours of referral and re-evaluations every 30 days, as appropriate, coordinating plan of care with the physician as needed.
• Prepares clinical and progress notes and required summaries, as appropriate.
• Revises plan of treatment and contacts the physician for change in plan of care.
• Advises and consults with patient’s family and other health care staff.
• Coordinates care with agency DON and other staff as appropriate.
• Observes, records, and reports any changes in the patient’s condition and updates the agency on changes in the patient’s status or schedule changes.
• Supervises the Physical Therapy Assistant.
• Instructs patient and/or family in the use of equipment, safe patient management, and home therapy programs as needed.
• Communicates effectively with patients, caregivers, and families as well as agency staff.
• Provides quality and appropriate care and reports issues or concerns to the DON/Administrator.
• Assures that all tasks are performed in a timely manner.
• Checks in on Physical Therapy Assistants every 4-6 patient visits.

Qualification

• Must be a licensed Physical Therapist by the state of Illinois.
• Must have at least one year of Home Health Care clinical experience.
• Car with required proof of insurance coverage.
• Maintains annual TB test, CPR certification, and other required.
• Able to read and interpret instructions related to patient care.
• Able to effectively perform and prioritize multiple functions or tasks as needed.
• Must be able to travel to prospective patient’s place of residence.
