Job summary

Imperial Healthcare Center in La Mirada, CA, is seeking a motivated Physical Therapist to join our rehabilitation team. You will work in a friendly environment with an experienced team and a convenient location with on-site parking.

Responsibilities include evaluating residents, developing individualized treatment plans, documenting progress, and collaborating with the rehab and nursing teams. New grads are welcome to apply.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current California Physical Therapist license.
• Strong clinical reasoning, communication, and teamwork skills.
• New graduates are welcome to apply.
• Skilled nursing facility experience preferred, but not required.

Responsibilities

• Evaluate and treat residents with orthopedic, neurological, and functional mobility needs.
• Develop and implement individualized treatment plans to improve strength, balance, mobility, and independence.
• Provide skilled physical therapy interventions to support rehabilitation and recovery.
• Complete accurate and timely evaluations, progress reports, and treatment documentation.
• Collaborate with rehabilitation team, nursing staff, and other interdisciplinary team members.
• Educate residents and families on mobility, safety, fall prevention, and therapeutic exercises.
• Participate in discharge planning and interdisciplinary care meetings.
