Responsibilities

• Evaluate residents' physical abilities, mobility needs, and functional limitations using standardized assessments.
• Develop and implement individualized treatment plans based on assessments and physician orders.
• Provide skilled PT interventions including therapeutic exercise, manual therapy, and modalities.
• Monitor and document resident progress and adjust treatment plans as needed.
• Coordinate care with physicians, nurses, OTs, and SLPs to optimize outcomes.
• Educate residents and families on goals, plans, and home exercise programs.
• Maintain accurate, timely documentation of PT services.
• Participate in team meetings, in-service education, and quality improvement initiatives.
• Stay current with PT practice and regulatory requirements.
