Job summary

Staff Physical Therapist position in Willoughby, Ohio, within an in-house therapy team. You will evaluate and treat residents, collaborate with clinical leadership, and maintain compliant documentation in medical records.

You will develop individualized treatment plans to improve mobility and function, supervise PT assistants, and educate families and staff on goals and home exercise programs. A state license and PT degree are required; a master/doctoral degree is preferred.

Qualifications

• Requires licensure as a Physical Therapist in the state.
• Bachelor's degree in Physical Therapy; Masters/Doctorate preferred.
• Proficient with clinical documentation systems (Net Health, PCC).

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility and function.
• Supervise Physical Therapy Assistants and support staff.
• Communicate progress with physicians, nursing, residents, and families.
• Complete timely documentation in accordance with regulatory requirements.
• Provide staff in-services and training for facility staff.
• Participate in quality assurance and rehab program development.
