Job summary

Evolve Therapy Services, LLC is hiring compassionate Physical Therapists for our partner in Mobile, Alabama, to provide in-house therapy at a Skilled Nursing Facility. Roles include evaluation, plan development, and interdisciplinary collaboration to ensure quality resident care.

We offer full-time, part-time, or PRN positions with opportunities for growth and ongoing licensure maintenance, including CMS-compliant documentation in NetHealth EMR.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Demonstrate sound judgment in evaluation, planning, and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention/treatment.
• Conduct timely screening of residents referred to physical therapy to determine need for intervention/ treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and team members.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR systems.
