Job summary

Evolve Therapy Services in Sevierville, TN is seeking a Licensed Physical Therapist to provide in-house facility services for residents. You will assess, plan, and coordinate therapy programs with an interdisciplinary team to meet CMS requirements and resident goals.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents.
• Adhere to documentation and billing procedures for PT services.
• Demonstrate sound clinical judgment in evaluation, planning, and follow-up of therapy programs.
• Screen residents to determine need for intervention and treatment.
• Develop treatment plans with short and long-term goals.
• Communicate findings and recommendations to residents, families, and the interdisciplinary team.
• Complete compliant CMS-relevant documentation in NetHealth EMR.
