Job summary

Evolve Therapy Services is seeking a dedicated Physical Therapist for an in-house Skilled Nursing Facility in Sevierville, TN. You will evaluate residents, develop treatment plans, and coordinate with an interdisciplinary team to deliver quality PT services.

Minimum BS in Physical Therapy, active state license, and commitment to ongoing CEU requirements are required. Position offers flexible full/part-time or PRN options with CMS-compliant documentation in NetHealth EMR.

Qualifications

• Bachelor of Science in Physical Therapy (accredited program).
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• Assume responsibility for ongoing professional development and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screenings of residents to determine need for intervention/treatment.
• Screen residents referred to physical therapy to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Conduct specialized evaluations as indicated.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings, treatment plan, and recommendations to residents and families/teams.
• Perform all other duties as assigned to discharge responsibilities in the best interests of the organization.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR systems.
