Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Owego, NY. Full-time, part-time, or PRN positions are available.

You will plan, organize, and direct physical therapy services, document per CMS expectations in NetHealth EMR, and collaborate with the interdisciplinary team to ensure high-quality resident care. Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in NY and other applicable states.
• Ongoing continuing education to maintain licensure and certification.
• Carry responsibility for ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and departmental procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Develop treatment plans with short- and long-term goals and implement them.
• Interpret evaluation findings and communicate recommendations to residents and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
