Job summary

Evolve Therapy Services seeks a licensed Physical Therapist for an in-house Skilled Nursing Facility in Owego, NY. The role supports full-time, part-time, or PRN positions, delivering patient-centered PT services in a facility setting.

You will evaluate residents, develop and monitor treatment plans, document care per CMS standards, and collaborate with the interdisciplinary team to ensure quality resident care and compliant processes.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• Commitment to professional development and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents per standards of practice and department procedures.
• Adhere to documentation and billing procedures for therapy services.
• Develop treatment plans with short and long-term goals.
• Communicate findings and plans to residents, families, and the care team.
• Complete CMS-compliant documentation in NetHealth EMR.
