Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Owego, NY. The role supports planning, organizing, developing, and directing therapy services compliant with federal, state, and local standards.

Responsibilities include evaluating residents, developing treatment plans, and ensuring accurate CMS documentation in NetHealth EMR. Active PT licensure and ongoing education are required; full-time/part-time/PRN options may be available.

Formación

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to ongoing professional development to maintain licensure/certification.

Responsabilidades

• Provide physical therapy services per standards of practice and department procedures.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate residents to plan and implement treatment and set goals.
• Develop treatment plans with short and long-term objectives.
• Communicate findings and recommendations to residents, families, and interdisciplinary team.
