Job summary

Evolve Therapy Services is a therapy management organization seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility partner in Clarion, PA. This full/part/PRN position focuses on delivering patient-centered PT care within established clinical and CMS guidelines.

The role requires a Bachelor of Science in Physical Therapy, an active license in the state(s) of practice, and ongoing professional development to maintain licensure.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and company procedures.
• Adhere to company procedures regarding documentation and billing of physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention or treatment.
• Evaluate residents to obtain data necessary for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals and methods to achieve identified goals.
• Communicate evaluation findings and recommendations to residents, families, and interdisciplinary team members.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR systems.
