Qualifications

• Bachelor’s degree in Physical Therapy from an accredited college or university.
• Must maintain an active, unencumbered license granted from state of employment.
• Must possess basic computer skills and ability to navigate through various software programs, including EHR.

Responsibilities

• Return patient to optimal physical function in a SNF setting and assist therapy goals.
• Instruct patients on maintaining function after discharge through exercises and activity modification.
